• Original, completed, signed Personal/Joint Bank account application form – Provided by the bank
• Signature Card -  provided by the bank
• Depository Agreement & General Terms & Conditions Agreement – provided by the bank
• Two letters of reference on the owner and/or signatories of the account – one reference must be from a bank, the other can be from a business professional (accountant, attorney, etc) or business associate.
• Notarized copy of the photograph page of the passport of the account holder and each authorized signatory.
